Outpatient Diabetes Education
The CHWC outpatient diabetes education program empowers participants to manage their diabetes successfully by teaching the necessary skills to help keep their blood glucose levels in target ranges. Our diabetes program is a recognized education program through the American Diabetes Association for meeting national standards for diabetes self-management education and support.
Individual and group sessions are available. Education sessions focus on general diabetes overview, medication information, nutrition & meal planning, exercise, blood glucose monitoring and goal setting.
Our staff includes two registered dietitian nutritionists (RDNs)—both of whom are certified diabetes care and education specialists (CDCES).
Our educators will provide management skills and motivational strategies to promote healthy life changes and prevent complications associated with diabetes.
Medicare and most health insurance providers cover the cost of outpatient diabetes education.
Patients must have a referral from their primary care provider to schedule an appointment.
