Hospital Hospice offers services for terminally ill patients and their family members, with the emphasis on pain control and symptom management. Our professional services are provided based on a referral from a physician that indicates a terminal diagnosis and that the patient is thought to be within the last six months of life.

 

The hospice team helps the patient and family with medical, spiritual, social, and psychological issues and assists with the legal paperwork concerning end-of-life care. Hospice nurses visit to evaluate pain and other symptoms. They arrange for medications to manage the symptoms and plan for any necessary medical equipment. Trained volunteers are a valuable asset to our hospice team and are available for companionship or respite care.

 

Hospital Hospice care for patients who reside in Williams, Defiance, Fulton, or Henry Counties. Hospice services are available to residents of most nursing care centers in the area. For patients who live at home, a primary caregiver to direct the care of the patient is a key element in hospice care.

 

Medicare, Medicaid and most insurance plans cover home health and hospice care services, including most medicines and medical equipment, but patients will not be excluded from care due to inability to pay.

 

Hospice services include:

  • Visits from members of the hospice team
  • Medication for pain and symptom management
  • Durable medical equipment such as oxygen, bed, walker, and wheelchair
  • 13 months of bereavement follow-up for families

 

Hospice is lead by an interdisciplinary team including:

  • Patient's physician
  • Hospital medical director
  • Registered and licensed practical nurses
  • Home care aides
  • Medical social workers
  • Counselors and clergy
  • Physical, occupational and speech therapists
  • Volunteers, family members and caregivers
