Hospital Home Health focuses on caring for patients in their own homes,
where many patients often respond and recuperate more quickly than in
another facility. Our professional services are provided based on a physician's
diagnosis and referral.
Hospital
Home Health has a team approach to caring for patients who need medical
attention at home. Our team includes nurses, physical therapists, occupational
therapists, and speech therapists plus social workers, dieticians, and
home care aides who are available when needed.
These
services are available on a part-time or intermittent schedule for patients
who live in Williams, Defiance, Fulton or Henry Counties. Assistance from
a a private duty aide is available for up to 24 hours.
If
a patient's needs change from home health care to hospice care, the transition
is easy. The same team of health care workers providers medical attention
for patients for home health and for hospice care.
Medicare,
Medicaid and most insurances cover home health care services that are
physician ordered when a patient is homebound.
